I called SCBN (May 2012) and spoke with Linda Campbell to discuss my prescription needs as well as monthly costs as follows:

1. Reviewed prescriptions :

o Folic acid, humalog insulin, lantus insulin, spironolactone, nadolol, moxatag, lyrica, xifaxan, and insulin needles (my wife & her daughter were witnesses during this call as I had you on speakerphone)

o You informed me that I was eligible for coverage on ALL medications for $100/month premium to SCBN with a one-time enrollment fee of $150.00

2. Received applications from SCBN, completed, and obtained prescriptions from Doctor, all of which was sent to SCBN July 2012. Timetable to receive medications was 4-6 weeks.

3. Experienced ongoing paperwork issues to include item #2 above based on the fact (per SCBN) that the pharmaceutical providers changed their application processes resulting in re-completion of said paperwork. In addition, there were other prevailing situations in which processing were delayed due to SCBN letters stating that the providers rules was that I was to mail the paperwork myself as it was not acceptable for SCBN to send it to them!

4. July 2012, communicated to SCBN regarding not receiving lantus insulin from Doctors office. SCBN informed me that the pharmaceutical company made an error, shipped, but did not have my name displayed on the shipment . SCBN (Cassidy) informed me that a new shipment was to arrive at my Doctors office. This communication was erronuos per Candace). August to current, experienced ongoing issues with receiving lantus insulin as my name was still not displayed on alleged shipments. I called SCBN, informed them of problems, and the response was SCBN cannot change the provider rules. If it was not for my Doctor giving me samples I would be destitute.

5. August 2012, I sent an email to SCBN (Candace) informing her that per Linda Campbellโ€™s phone conversation (May 2012) I was to receive medications annotated in #1 above. Candace emailed back stating that I was only eligible for Lantus, lyrica, Xifaxan, and precose (folic acid).

6. August 2012, I received a letter stating that I was not eligible for precose by the provider.

7. October 2012, I received first shipment of Xifaxan after waiting 4 months!

8. September 2012, I received first shipment of lyrica after waiting 3 months!

9. January 2013, I called provider for lyrica as I did not receive refills. The Company informed me that my refill expired December 4, 2012 and that I need to have my Doctor send over another prescription to process refill.

10. January 2013, I received a letter from SCBN informing me that I need to get another prescription from my Doctor for lyrica refill (over a month from the expiration date (item 9) above.

11. February 2013, I called provider (lyrica) again regarding refill. The Company informed me that they mailed the paperwork back to me as I need to send in another photo of my Drivers License. Itโ€™s been 2 weeks now and I still have not received the paperwork!

Due to SCBN inability to properly represent and deliver products and services through its relationship with pharmaceutical providers coupled with its inability to resolve ongoing issues, I stopped payment to SCBN as of January 2013. However, SCBN chose (unauthorized transaction) to charge my Social security benefit card with itโ€™s adjusted fee of $80.00 in January. This is a violation of section 207 of the Social Security Act (42 U.S.C. 407) thus I should receive a refund from SCBN.

12. Based on the onset date May 27, 2012 (item #1) above, SCBN did not execute its bilateral agreement to cover prescriptions. In fact, coverage (eligibility) was reduced from 9 items to 4 then to 3 eligible prescriptions.

As a result of SCBN breaching the bilateral agreement based on above facts, I incurred excess โ€œout of pocket expenses for required monthly medications.โ€ This violation is supported by Tort Law should SCBN choose to pursue this matter in the court of law.

I hereby request of SCBN to reimburse me for the $150 enrollment fee as well as monthly service fees since June 2012 (pro-rated for partial medications (Xifaxan & Lyrica) received).

Failure of SCBN to take immediate action will result in media coverage on false advertising by Company and itโ€™s inability to โ€œdo as they sayโ€ per this letter. In addition, effective immediately, bilateral agreement with SCBN is hereby terminated.

In addition to paying SCBN, incurred over $1000 for out of pocket cost for monthly medications. Company response was "sorry we missed you as a customer!" Please...reimburse me fees paid to you *** it!

Monetary Loss: $1410.

I just interviewed with them this past week for a position in sales. I must say after seeing their cramped call center and shabby office furnishings none of this surprises me.

I wanted a sales job where I could help people. I thought this was the one. I am so glad I researched them before my start date (tomorrow) lol.

I hope this all worked out for you folks.

Oh now it makes sense why they have a scan card entry door and basically a security guard at the front desk.

Companies like this need to be shut down.

I was going to suggest the same organization, pparx.org. They are wonderful, helpful people.

I have never paid a PENNY to them. They went out of their way trying to find a program for one medication (unable on that one which is about $1.50 per pill), but through a mfg were able to get me FREE migraine medication which retails for $10-15 PER PILL. They must be supported by drug co's, doctors, or other benefactors, I don't know. All I know is I don't mind being put on hold while they look through everything trying to find a deal.

Also, just in case you CAN'T find a better deal through pparx.org, you can use Costco pharmacy (no membership req'd) their prices are below cost; also if you have a AAA membership many retail pharmacies give a significant discount (the one I pay $1.50/pill for is with that discount, that is a recent increase by the mfg unfortunately, but better than straight retail or even so called "discount" cards. SCBN has misled you. SCBN is not an insurance company and they have no control over whether or not you qualify for free/reduced medication. The medication you received did not come from SCBN, it came from the patient assistance program run by the drug manufacturer.

You can access these programs on your own, free of charge, at www.pparx.org This link takes you to the Partnership for Prescription Assistance. Montel Willians is the spokesperson... you may have seen advertisements on TV.

I hope you take these people to court. I can't believe they are charging people a set up fee plus $25/month for each medication they receive help with. All SCBN does is fill out the paperwork for you which is something most people can do on their own. If, for some reason, you can't fill out the paperwork on your own, try to find a family member, friend, senior center, church, etc. to help you out.

SCBN tells a bucket full of lies regarding prescription assistance programs. They are robbing people, not helping them out.
